RN Clinician Community Placement Program

Full-time position available at our San Diego (Kearny Mesa) office

RESPONSIBILITIES

Under the direction of the Community Placement Program (CPP) Program Manager, the CPP Nurse supports individuals with complex needs transitioning from secure facilities into community settings. These individuals may be involved with multiple systems and present with a range of intellectual, developmental, mental health, and medical conditions. The CPP Nurse conducts clinical assessments, performs quality‑assurance visits to CPP residential homes, and identifies the healthcare and developmental needs of those served. The role also provides clinical guidance to the CPP team in developing health‑related goals for Individual Program Plans and assists vendors in creating Restrictive Health Care Plans.

​QUALIFICATIONS:

EDUCATION

Graduation from a school of nursing approved by the National League for Nursing; a Bachelor’s degree in Nursing is preferred. A valid license as a Registered Nurse in the State of California is required.

​EXPERIENCE

Two (2) years of Nursing experience in a health care setting. Experience in working with individuals with a developmental disability is preferred.

​​OTHER

Ability to solve problems with minimal supervision; function within an Interdisciplinary or Planning Team setting; prepare clear and concise reports. Knowledge of normal and abnormal growth and development; effect of cultural and social patterns on health; organization and functions of social, health and other support service agencies; current nursing practice methods and procedures; and state and federal legislation related to service for the developmentally disabled.

​STARTING SALARY

$39.69 to $43.66 an hour
